regarding interest and request Relief Effort Forms or download
all forms by clicking on links below and mail to GCFC address.
2. Upon receiving Relief Effort Forms please fill out ALL
information and mail to GCFC at

or FAX at (409) 986.9043
3. Within one week of receiving your FORMS you will be
contacted by GCFC concerning confirmation of dates,
travel arrangements and other pertinent details.
PLEASE NOTE:
Relief Effort Forms MUST be received by
GCFC
before further process can take place.
